Making an Appointment
We strive to provide you with the medical care that you need, when you need it, as comfortably and as conveniently as possible.
To make an appointment, simply call our office during normal business hours. The number is
You select your primary care physician. Your primary care physician provides your regularly scheduled medical care and will also provide care for illnesses or injuries that arise between scheduled visits, wherever possible.
All of our health care providers are trained and certified in the diagnosis and treatment of medical problems.
If you are ill, and your primary care physician is not available, another medical provider will evaluate and treat you. If the problem requires further care, you will be given an appointment to follow up with your primary care physician. Your complete medical record is available to each provider within Community Care Physicians, so all are informed with the information they need to care for you.
Canceling an Appointment
If you must cancel an appointment, we ask that you do so within a day of the appointment. We would be happy to reschedule your appointment.
You will receive a call from our automated confirmation system two days before your scheduled appointment to confirm your appointment. If you are unable to make your appointment, please contact our office.
A fee is charged for patients who miss their appointment and do not call to cancel.
No Show Fee
If you miss an appointment without providing proper notice, another patient is prevented from receiving care. Therefore, Clifton Park Family Medicine reserves the right to charge patients a fee if you miss an appointment and do not call to cancel ("no show"). "No Show" fees will be billed to the patient. This fee is not covered by insurance, and must be paid prior to your next appointment. Our no show fee is $50.

Please call our office first before coming in if you are ill to avoid the potential of unnecessarily exposing other patients and our staff to the novel coronavirus (COVID-19). While we don’t currently offer walk-in hours, our practitioners are easily accessible to care for you in multiple ways, including over the phone, using a video visit (telemedicine), or in the office while following strict infection control procedures to keep you safe. Whatever your need, you should call our office and our team will determine the best way to care for you.
