Physician Hospital Affiliations
For over 30 years, Community Care Physicians, P.C. has focused on providing our patients easy access to timely outpatient medical services in an effort to avoid hospitalization by promoting preventive healthcare services and by better managing chronic conditions. However, at some point, you may require hospital care.
Our subspecialists care for patients in many of the upstate New York area hospitals. If you are a primary care patient and need hospital care, during your hospital stay a hospitalist may provide your inpatient care. Hospitalists are physicians whose primary professional focus is the general medical care of hospitalized patients.
Many of the physicians of Community Care Physicians have adopted the widespread practice of relying upon Hospitalist Physicians at each of the area hospitals to care for our patients when our patients are hospitalized. If this is the case for your Community Care doctor’s office, please know that we have established relationships with these physicians so that we will exchange important medical information promptly when our patients are hospitalized. This will insure continuous, uninterrupted medical care.
Please use the below list to determine with which hospital each physician has a relationship, in case you have an emergent need in the future. Be assured that, if you are hospitalized, we will see you promptly in our office after you leave the hospital.
