- Sometimes medical needs arise after hours when your doctor’s office is closed. Community Care has 2 Urgent Care Centers open evenings, weekends and holidays to treat non-lifethreatening medical conditions. These Urgent Care locations offer convenient, quality medical care when your regular doctor isn’t available. Since these Urgent Care Centers are a part of Community Care, your primary care doctor will be able to access the information on the care that was given in your electronic health record (EHR) that is shared by Community Care providers . - Sometimes it becomes necessary to involve a specialist in your care. Community Care includes a number of subspecialty physicians, all of whom share electronic health records with our primary care providers. This ensures that your medical records are accurate, safe, and secure while giving the providers administering your care the access they need to offer you the best possible care. We are happy to refer you to any of these specialists when needed or to any appropriate specialist you prefer.
