You must be at least 16 years of age and a patient of Community Care Physicians, P.C. to enroll
(A community care patient is anyone who has been seen at any of our office locations.
Upon enrollment we will verify that you are a patient).

Anyone under 18 must be accompanied by an adult (who is also a Workout gym member).

*The discounted rate is available to patients who commit for a 12 month period.
The rate for a 6 month commitment is $18.99/month.
