What should I bring to my urology appointment?

  • Your current medical insurance identification card.
  • A list of current medications.
  • Any radiology films or CDs and any biopsy slides. Your physician will not send these; you must bring them with you to your appointment.
  • Drivers license or photo identification
  • New patient packet – click here to download forms
  • Co-pay – Please refer to your insurance company for proper payment. We accept cash, credit/debit cards, or checks

Parking at our offices is free and readily available.
