Preparing for Your Visit

Please bring with you the following items so that we can serve you better.

• Insurance card. Please make sure that you have notified your insurance company that a physician at Albany Family Practice Group will now be your primary care physician.
• Co-payment. Your co-payment is due at the time of service. Please refer to your insurance company with any co-payment questions.
• Medical records from your previous physician.
• A list of current medications.

Please arrive promptly for you appointment so we can update your personal information if necessary. If you have changed your address or phone number, please let the front desk know at the time of your appointment.

Prescription renewals and referral requests that are not completed at the time of your visit require 48 hours notice.

If you have an outstanding balance on your account, you are required to pay that at time of service.
