New "Patient Connector" at Community Care Physicians P.C. Makes Finding the Right Primary Care Doctor Easier, Faster. Patients can now call one phone number, the Patient Connector, to establish as a new patient with one of Community Care Physicians' 18 Primary Care offices.

Community Care Physicians, P.C. (CCP), the largest multispecialty medical group in the Capital Region of NY, has implemented a system that makes it easier for a new patient to schedule an appointment with a primary care physician within their group. CCP's new "Patient Connector" will work closely with patients who are looking to establish with a new primary care physician, including physicians in Internal Medicine, Pediatrics and Family Medicine. Instead of calling multiple offices and compiling information on each individual provider, now a patient can simply call a centralized number at CCP, 518-640-6729, and the Patient Connector will help them find the right doctor, making it faster and easier for area residents to locate a new primary care physician for themselves or their family. This is a free service to patients.

The Patient Connector takes patients through series of questions, including but not limited to, where the patient lives, how far they are willing to travel, if they need a doctor for themselves or their children, and whether they prefer male or female. The Patient Connector then schedules an appointment for the patient with the physician that is chosen. Any calls to the Patient Connector are returned within 48 business hours.

Currently, the Patient Connector is scheduling appointments for patients at 18 primary care offices of CCP, including: Albany Family Practice Group, Latham Medical Group, The Office of Drs. Fruiterman, Greenblatt and Maier, The Offices of Drs. Bertram and Patil, the Office of Dr. Lev Barats, Latham Internal Medicine, Partners in Family Medicine- Troy, Partners in Family Medicine - Cropseyville, Clifton Park Family Medicine, Community Care Family Medicine, Saratoga Medical Associates, Niskayuna Internal Medicine, Schodack Internal Medicine and Pediatrics, Burnt Hills Pediatrics and Internal Medicine, Capital Healthcare Associates, Community Care Pediatrics-Clifton Park, Community Care Pediatrics- Saratoga, Community Care Pediatrics-Latham.
