Our doctors have been providing care to children in the Saratoga, NY area for over 60 years. Our physicians began practicing years ago in their own individual practices in Saratoga Springs, NY.  In the early 80’s, these physicians merged together and called themselves Saratoga Medical Associates. Again, in the 90’s, the physicians merged with Community Care Physicians and became Community Care Pediatrics - Saratoga.

Our pediatricians are board certified specialists with years of experience providing medical care for children from infants through teenage years. We thank you for the privilege of caring for your children.

Locations and Hours

One of our priorities is to make sure your medical care is easily accessible. So, Community Care Pediatrics-Saratoga has extended office hours for our patients. One of our doctors is on call each evening and weekend.

We have what we call “Walk-In” hours. These are set times each day when our established patients can arrive to the office without an appointment to be seen for sudden illnesses such as sore throat, fever, and/or earaches.

Connect With Our Office Online

Community Care Pediatrics-Saratoga has implemented a patient portal to provide patients with a new option for connecting with their doctor. This portal, called mycareDOT, offers patients and physicians a wide array of services to facilitate health information exchange, including: personal health record (PHR), requests for appointment scheduling and referrals, patient health education and electronic prescribing.

To enroll in mycareDOT, you must first be an established patient of our practice. Simply click here or visit www.mycaredot.com. After setting up a new user account, they then proceed through the set up wizard, where you will be prompted to choose your child's pediatrician with whom you will communicate. From there you can update your child's personal health record, including medical problems, medications, allergies, family and social history, and immunizations.

In mycareDOT, before you can communicate with the doctor's office, the physician must first accept or decline the online relationship with you. Once accepted, you can send general messages to their doctor, request/cancel an appointment, request a prescription refill, or request a referral. All of these services are free of charge. From the comfort of your own home, office or any location with Internet access, you can now communicate with our office at any time for routine questions.

Emanuel Nicholas Cirenza, MD, FAAP

Dr. Emanuel Cirenza graduated from Siena College with a B.S degree in Biology and Chemistry in 1990 and received his medical degree from the SUNY Health Science Center at Syracuse in 1994. He completed his internship and residency at the Eastern Virginia Medical School in Norfolk, Virginia in 1997. During that same year he joined Community Care Physicians' pediatric office in Saratoga and still practices there today.

He has been Board Certified by the American Board of pediatrics since 1998 and he is a member of the American Academy of Pediatrics.

Dr. Cirenza is active in the Saratoga Springs community where he works year round on behalf of children's literacy programming, serves on the Board of the Saratoga Center for the Family and is on the Board of Wilton Youth Baseball. Dr. Cirenza is a recipient of Community Care Physicians' 2004 Caring for Our Community Award in recognition of his outstanding volunteer efforts in support of children's literacy, youth Soccer and Baseball.

When not working or volunteering he enjoys hiking and participating in triathlons. Dr. Cirenza lives in the Greater Saratoga region with his wife and three children, Allison 15 yrs old, Ian 13 yrs old, and Sam 9 yrs old.

Jami Marie Hawthorne, MD, FAAP

Dr. Jami Hawthorne graduated from St. Lawrence University, Cum Laude in Biology with a B.S degree in 1988. She attended SUNY Buffalo for Graduate work in Oncology and received her medical degree from the University of Vermont College of Medicine in 1993. In 1996 Dr. Hawthorne completed her pediatric residency as Chief Resident at the University of New Mexico Health Science Center.

Dr. Hawthorne has been Board Certified by the American Academy of Pediatrics since 1997. She is a member of the American Academy of Pediatrics, the Medical Society of New York State and the Jefferson County Medical Society.

Dr. Hawthorne practices with Community Care Pediatrics in Saratoga and is actively involved in the Saratoga Community. She speaks Spanish fluently and has a special interest in working with her patients and their families to assure that all children have the opportunity to reach their fullest mental and physical potential. Dr. Hawthorne lives in the Greater Saratoga region.


Stephanie Jorgensen, MD, FAAP

Dr. Jorgensen received her BA degree from Siena College in Loudonville and her medical degree from Albany Medical College in Albany, New York. She completed her residency as Associate Chief-Resident at Albany Medical Center Hospital from 1997 to 2000. Dr. Jorgensen is Board Certified by the American Board of Pediatrics and belongs to the American Academy of Pediatrics, MVMA Physicians and is an Instructor for the Neonatal Resuscitation Program.

Dr. Jorgensen worked at Glens Falls Pediatrics from 2000 to 2010. During this time she was also the District Physician for Lake George Central Schools. She began working at Community Care Physicians September 2010.


Laura Yates LeCours, MD, FAAP

Dr. LeCours graduated from Mount Holyoke College Magna Cum Laude and Phi Beta Kappa with a B.S in Biochemistry in 1981. She received her medical degree at Albany Medical College, Cum Laude in 1985 and completed her pediatric residency and internship at Albany Medical Center Hospital in Albany, New York in from 1985 to 1989. She has been Board Certified by the American Academy of Pediatrics since 1989.

Dr. LeCours was the Chairperson of the Pediatrics Department in Saratoga Hospital from 1992-1994. From 1994 to 1989 she practiced pediatric medicine with Dr. Charles Richman in Saratoga Springs. At the same time in 1994 she began her affiliation with Saratoga Medical Associates in Gansevoort, NY. She has been a Pediatrician at this site ever since.

Dr. Lecours is affiliated with Saratoga Hospital and Four Winds; both located in Saratoga Springs, New York. She is a member of the AMA, the American Academy of Pediatrics and the Saratoga County Medical Association.

Dr. LeCours has special clinical interests in the areas of infectious disease and nutrition. She is married with two children, an active and talented member of her church choir, and has been living and practicing in the Saratoga area for more than ten years.


Anne Katherine Marthy-Noonan, MD, FAAP

Dr. Marthy-Noonan graduated from the College of St. Rose, Cum Laude with a B.A. 1975. She received her medical degree at Albany Medical College, in 1980 and completed her pediatric residency and internship at the United States Navy Hospital in San Diego, California from 1980 to 1984. She has been Board Certified by the American Academy of Pediatrics since 1985 and is certified in both Basic Life Support and Advanced Life Support since 1979. She is a Fellow in the American Academy of Pediatrics.

In 1981 Dr. Noonan began her career as a General Medical Officer on the USS Dixie AD14 in San Diego, California. She then became a Staff Physician in the Department of Pediatrics at the Naval Hospital in Groton Connecticut from 1984 to 1986. From 1990 to today Dr. Noonan has worked with Community Care Physicians as a Pediatrician in the Saratoga (Gansevoort) office.

Dr. Noonan brings a comprehensive approach to the health of children she cares for and works with parents to promote literacy, physical fitness and healthy nutrition for their children. She cares for children from infancy through college age and has lived in the Saratoga area for close to 25 years.


Charles Henry Richman, MD, FAAP

Dr. Richman received his Bachelors degree from Duke University and his medical Degree from the University of North Carolina, Chapel Hill, North Carolina. He completed his internship at Chapel Hill and pediatric residency at the University of Virginia in Charlottesville.

Dr. Richman had a pediatric private practice in Saratoga Springs for twenty years. In 1994 he became a member of Saratoga Medical Associates, PC in Gansevoort, NY and continues to practice there today.

Dr. Richman is a certified by the American Board of Pediatrics. He is a member of the American Medical Association and the American Academy of Pediatrics He cares for children from infancy through college years and provides a comprehensive approach to the health and development of children under his care.

Dr. Richman enjoys hiking and cross country skiing and has lived and worked in the Saratoga area for the duration of his career.

Jennifer A Shulof, PNP

Jennifer Shulof received her BS degree, and completed her Pediatric Nurse Practitioner Program, earning her MS Degree from Syracuse University. She holds a Registered Nurse and Pediatric Nurse Practitioner license as well as an American Nurses Credentialing Certification. Her professional memberships include the National Coalition of Nurse Practitioners and the National Association of Pediatric Nurse Associates and Practitioners.

Jennifer has teaching experience at several institutions as a Clinical Preceptor, Lab Instructor and Clinical Instructor. Ms. Shulof worked at as a Staff Nurse in Rochester and Syracuse from 1993 to 1996. In 1996 she began her career as a Pediatric Nurse Practitioner and eventually moved to this area in 2001, working at Four Seasons Pediatrics in Clifton Park, Adirondack Pediatrics and finally Community Care Pediatrics in Saratoga in 2009.

Our Services

Our providers and staff are proud to work in a field specifically dedicated to the care of people during a very important stage of life- childhood. Pediatrics is the medical specialty entirely focused on the physical, emotional, and social health of children from birth through adolescence. The focus of pediatrics is on preventive health care and the proper development of children.

Pediatricians are also recertified periodically to keep current on changes in children's healthcare.

Our physicians are committed to the healthcare of children in Saratoga. Our doctors are the school physicians for the Saratoga School District. They complete physical exams on site at the schools and they are present for the home football games in case of injury or incident.


Hospitalization

If an emergency arises and your child needs to visit the emergency room your child's primary care physician or the physician on-call will be able to direct you to an appropriate emergency room based upon their symptoms. Both Albany Medical Center Hospital and Saratoga Hospital are utilized by the providers at Saratoga Pediatrics. In the event of a hospitalization, your child would be cared for by the AMCH Hospitalist group and in Saratoga Hospital, the Saratoga Inpatient Physician Group.

Rest assured, your primary care physician will be kept informed about your child's medical status.

Your Visit


Patient Preparation

A little preparation will help us to serve you better. Bring to your child's appointment their health insurance identification card and your pharmacy plan insurance card. Bring also a form of picture identification, like a driver's license (yours, if your child is under the age of 18)

If your child is taking medications, either prescription or non-prescription, bring their pill bottles to our office.

Come to the office about 15 minutes before their scheduled appointment so that we can update your child's personal information in plenty of time.

Please check-in at the reception desk as soon as you arrive.



Making An Appointment

We strive to provide your child with the medical care that they need, when they need it, as comfortably and as conveniently as possible. All of our health care providers are trained and certified in the evaluation and treatment of all aspects of pediatric health care. To make an appointment, simply call our office during normal business hours at 518-584-0355.

You select your children's primary care physician. Their primary care physician provides them with regularly scheduled medical care and will also provide care for illnesses or injuries that arise between scheduled visits.

If your child is ill, and their primary care physician is not available, another Pediatric provider in our practice will evaluate and treat him or her. If the problem requires further care, you will be given an appointment to follow up with your child's primary care physician. Their complete medical record is available to each provider within Community Care Physicians, so all are informed with the information they need to care for your child.



Cancel An Appointment

If you must cancel an appointment, we ask that you do so at least one day before the appointment. We are happy to reschedule your appointment.

We call every patient's contact phone number the day before their appointment to verify that they will attend.



Waiting Times

The medical provider makes every effort to see you and your child on time. This is usually within 15 minutes of your appointment time.

Occasionally your provider may be delayed. If so, it is because the health condition of other patients has required more time than your provider anticipated.

Our providers will afford each patient the time that is needed to attend to the health problems at hand. All patients will receive the same attention.

Please be patient if the provider is delayed. If the provider is occupied with a serious health emergency, you will be given the options to wait for the provider, to see an alternate provider, or to reschedule the visit.

If you have not been called from the waiting area within 15 minutes after your scheduled appointment, please return to the reception desk.



Prescriptions

Prescription medications can be an important part of your plan of care. To dispense your child's medications safely and effectively, follow a few simple suggestions.


Pill Bottles

Please bring all your child's pill bottles with you to EACH appointment so that we can verify the names and doses of their drugs, their expiration dates and number of refills available. Keep all medications in accurately labeled bottles or containers.


Refills

We want to refill at your office visit any prescriptions that will expire before your child's next scheduled visit. (Bringing their pill bottles helps us to determine which medications need refills).

If you call the office to refill a prescription that expired between visits, please call during normal business hours, and allow 48 hours for the physician to review your child's medical record and provide the prescription.

Depending on your drug plan and the drug, your prescription may be faxed directly to the pharmacy, or you may need to pick-up the prescription at the office.

Please Note: Photo ID is required when picking up scripts for controlled substances.


Controlled Substances

Controlled substances (narcotics, sedatives, stimulants and the like) require special handling. Refills are limited, and in some cases are not permitted by law.

All prescriptions for controlled substances must be picked up in person with a photo ID at our office by the person responsible for the child for whom they are prescribed, or by their designated health care proxy.

Community Care physicians are not permitted to provide prescriptions for controlled substances by telephone when they are on-call.



Referrals

Your primary care provider may recommend that your child be evaluated and treated by a specialty physician if their medical condition warrants.

Community Care Physicians includes a number of excellent specialty and subspecialty physicians, all of whom share electronic health records and imaging studies allowing your child's records to be immediately accessed by the providers administering their care. We are happy to refer you to any of these Community Care specialists when needed or to any appropriate specialist you prefer.

Our office will provide notifications to, or request authorization from your health insurer if these are required.

We will provide the consultant with your child's relevant medical history so that he or she is aware of your child's medical problems.



Co-Payments And Balances

Payment is expected at the time of service.

If your insurance requires that you pay a co-payment, they require us to collect it. For your convenience we accept cash, checks and most major credit cards. If we must bill you for the co-payment, an additional processing fee is charged.

Please complete these forms prior to your scheduled appointment and mail them back to our office. If your appointment is scheduled in fewer than 5 days, please fax the forms to our secure fax, 518-583-7665, or bring them with you to your scheduled visit.
