Naomi Bloomfield, MD
Dr. Bloomfield is Board certified by the American College of Obstetrics and Gynecology and is a Fellow of the American College of Obstetrics and Gynecology. She received her Bachelor of Arts degree in Biology with a Chemistry minor from Russell Sage College in Troy and her Master of Science degree in Toxicology and Doctor of Medicine degree from Albany Medical College. Dr. Bloomfield serves as clinical assistant professor at the Department of OB/GYN at Albany Medical Center. She has also presented research on women’s health topics at a number of conferences throughout the country. Dr Bloomfield is a member of the American Institute of Ultrasound in Medicine. She has special interest in adolescent gynecology, laparoscopic surgery (especially laparoscopic hysterectomy), menopausal medicine and urodynamics. Dr Bloomfield will be the new chairman of the new Burdett Care Center at Samaritan. Dr. Bloomfield practices in Clifton Park, Rensselaer, and Troy.
Christopher Bloss, MD
The experiences my wife and I shared in the births of our six kids have helped shape my philosophy and perspective as an obstetrician. Believing that birth is a natural event has enabled me to coach women through the delivery process; throughout a patient's pregnancy I am happy to devote whatever time is needed for communication and information. On the gynecological side of things, I have trained to be able to provide state of the art techniques such as outpatient robotic & laparoscopic hysterectomy and hysteroscopic sterilization that allow my patients to get back an their feet and to their busy lives. I have a special interest in the evaluation and treatment of women with chronic pelvic pain conditions. Evidence based patient care and a positive doctor-patient collaboration are always my most important goals.

Steven Deserre, CNM
Steven is a native of northern NYS, having relocated to the Capital region in 1987 after living and working in Houston, Texas for several years. He developed an interest in obstetrics and women’s health care after witnessing the birth of his first child in 1980. Upon graduation in 1981 Steve commenced working in labor and delivery at several hospitals over the next 20 years in various levels from low risk to Albany Medical Center’s level 3 transfer center. This long love of OB nursing fostered the start of his current career as a Certified Nurse-Midwife (CNM) in 1998. Being a CNM has allowed Steve to participate in the whole childbearing process; as well as, promoting women’s GYN wellness and family planning needs. He sees his role as providing safe, effective, comprehensive and professional care in support of a woman’s physical, emotional and educational/ informational needs with respect to their spectrum of health care from birth to menopause.
Steven resides in the Glenville area with his wife of 33 years, Lisa and three feisty Chihuahuas. He is the father of 3 young adult children, 2 married daughters (Megan & Courtney) and a son (Alec) in college. He became the proud grandfather to a grandson, Lucas last year, and is enjoying this new role like no other.
Steven holds a diploma in Registered Nursing from St. Lawrence Psychiatric Center School of Nursing and Bachelor’s of Science in Nursing from Excelsior College. He holds a certificate in Certified Nurse Midwifery from Frontier Nursing University.
Mary Elacqua, MD
Dr. Elacqua is Board certified by the American Board of Obstetrics and Gynecology and is a Fellow of the American College of Obstetrics and Gynecology. She received her Doctor of Medicine degree from Universita Degli Studi Roma in Rome, Italy and then returned to the Capital Region to complete her internship and residency training in the Department of OB/GYN at Albany Medical Center Hospital. Dr. Elacqua is the current Chairman of the Ob/Gyn Department at Samaritan Hospital. She has held this position for the last six years. Dr Elacqua has special interests in menopause management, adolescent gynecology, advanced pelviscopic surgery as well as botanical and herbal medicine. Dr. Elacqua practices in Clifton Park, Rensselaer, and Troy.

Dr. Ford completed his Bachelor of Science degree at Harvard University and SUNY at Buffalo, where he went on to receive his Doctor of Medicine degree from the SUNY at Buffalo School of Medicine. Dr. Ford completed his residency training in the Department of Obstetrics and Gynecology at Albany Medical Center Hospital. Dr. Ford is a clinical instructor in obstetrics and gynecology at Albany Medical College. He has special medical interest in female urinary incontinence, infertility, minimally invasive surgical procedures and office based sterilization for the disadvantaged and third world countries. Dr. Ford is a member of the Society of Obstetricians and Gynecologists of Canada. Dr. Ford practices in Clifton Park, Rensselaer, and Troy.
Michelle Gallant, CNM
Michelle Gallant is certified by the American Midwifery Certification Board. She received her Bachelor of Science degree in Administration of Health Services from Ithaca College; her Associate of Science degree in Nurse Education from the North Shore Community College in Beverly, MA; and her Certificate in Midwifery from Baystate Midwifery Education Program in Springfield, MA. Michelle also works as a Sexual Assault Nurse Examiner for the Massachusetts Department of Public Health and serves as Clinical Faculty of the SANE Certification Program.
Michelle and her husband, all together have five grown children. They have a Lhasa Apso they adore and who rules the house more than any of the children ever did. In her leisure time, Michelle enjoys summers boating on Sacandaga Lake with her husband and in the many cold weather months of the Northeast they both enjoy to travel to warm weather destinations. In addition, Michelle loves to shop and will travel anywhere new just to go shopping somewhere new.

Robert Walter Kelty, MD
A native of Long Island, Dr Kelty earned his undergraduate degrees from Suffolk Community College and SUNY Stony Brook. Afterwards he received his Doctorate from NY State Program at The Sackler School of Medicine of Tel Aviv University.
His residency in Ob/GYN at the Albany Medical Center Hospital, brought him to upstate New York. After meeting his wife Colleen, a registered nurse from Delmar, he decided to plant roots in the Capital District.
Upon graduation he worked for St. Peter's Hospital for eight years, then opened a thriving solo practice in Delmar for over a dozen years. He joined Community Care Physicians in 2013 because of the opportunity to work in a large multispecialty group setting.
Office practice with an emphasis on the continuum of care from adolescence through menopause. Specializing in cancer screening and osteoporosis.
He resides in Guilderland, with Colleen and their four children and two dogs.

Dr. Medina is an upstate native. He grew up in Syracuse, NY where he attended LeMoyne College and then Upstate Medical University. He then completed his OB/GYN residency at Upstate Medical University. He has a daughter in elementary school, as well as a dog named Lucy. Dr. Medina spent four years practicing medicine with the US Air Force and was stationed in Biloxi, MS. While his daughter keeps him busy, he enjoys skiing in the winter and traveling.
