Kristine Janice Campagna, DO

Kristine Campagna, DO grew up in Garden City, NY. She is Board Certified in Family Medicine and has a Certificate of Added Qualification in Sports Medicine. Dr. Campagna received her Bachelor of Arts degree from Columbia University and her Doctor of Osteopathic Medicine from New York College of Osteopathic Medicine (NYCOM). She completed her osteopathic rotating internship and her residency training in Family Practice at Albany Medical Center. She then completed a Primary Care Sports Medicine Fellowship at The Ohio State University. As a provider who enjoys the practice of medical principles related to the science of sports, it's not a surprise that she also enjoys sports outside of the office. Dr. Campagna likes running, weight training, and spending time with her husband and 2 sons.

Raymond Joseph Carrelle, MD

Raymond Carrelle, Jr, MD is board certified in Family Medicine. He received his Bachelor of Science degree in Biology from the University of Scranton. Dr. Carrelle then received his Doctor of Medicine degree from the University of Miami School of Medicine in Miami, Florida. He completed his residency through the Albany Medical Center Family Practice Residency Program. Dr. Carrelle also received his Masters of Business Administration from Rensselaer Polytechnic Institute in Troy, NY.

Denis Ernest Chagnon, MD

Denis Ernest Chagnon, MD

Dr. Chagnon, originally from the Albany area, did his undergraduate work at Georgetown University and graduated from Georgetown Medical in Washington DC, then did his Residency in Family Practice in Syracuse, NY before joining the Latham Medical group where he has practiced the full spectrum of family medicine since 1975. Board Certified (and recertified) in Family Medicine, he is also a Fellow of the American Academy of Family Physicians (AAFP).

A past-President of the New York State Academy of Family Physicians, Dr. Chagnon currently serves on the Board of the AAFP Foundation. He lives in Niskayuna with his wife Brita. They have two married sons and their first granddaughter was born in 2010. He remains an active golfer, runner, cross-country ski marathoner and Yankee fan.

Ranen Chakraborty, DO

Ranen Chakraborty, DO

Dr. Chakraborty grew up in Queensbury, NY. He is Board Certified in Family Medicine. As a Doctor of Osteopathic Medicine, Dr. Chakraborty brings additional benefits of osteopathic manipulative techniques to diagnose and treat patients. Dr. Chakraborty received his Bachelor of Science degree in chemistry from Union College of Union University and his Doctor of Osteopathic Medicine from New York College of Osteopathic Medicine. He completed his residencies in Miami Children’s Hospital, St. Claire’s Hospital and Health Center in New York, Mercy Hospital of Pittsburg, and UPMC Shadyside Hospital in Pittsburgh. In addition to primary care, Dr. Chakraborty has a special interest in Urgent Care. He enjoys skiing, tennis and movies. Dr. Chakraborty welcomes new patients and practices at Latham Medical Group and Community Care Physicians’ Urgent Care Centers.

Holly Krick Cleney, MD

Holly Cleney, MD is board certified in Family Medicine. Dr. Cleney grew up in small towns in the Catskill Mountains (Andes and Delhi), NY. Dr. Cleney attended SUNY Binghamton for college, where she received her Bachelor of Arts Degree and where she also played basketball (her skills can be seen in Community Care’s 2009-2010 “Connecting the Dots” commercial!). She then received her Doctor of Medicine degree at SUNY Health and Science Center at Syracuse. Dr. Cleney completed her residency training in family practice at Albany Medical Center. She has 2 children and a dog at home.

Elizabeth Anne Locke, MD

A graduate of SUNY Upstate Medical University at Syracuse, Dr. Locke completed her internship and residency training in family medicine at Albany Medical Center, where she also served as Chief Resident. Dr. Locke grew up outside of Syracuse, NY. She plays ice hockey, softball and enjoys downhill skiing and hiking with her husband and dog. Dr. Locke has a special interest in women’s health and performs many gynecological procedures including PAP smears, colposcopy, IUD, and endometrial biopsy.

Dr. Ludwig is a Diplomat of the American Board of Family Medicine. She received her Doctor of Medicine degree from Albany Medical Center, where she also completed her residency training in family medicine. Dr. Ludwig is a mother of 2 boys and enjoys knitting/crafting and running (completed numerous triathlons, marathons, and races), biking and outdoor activities. Dr. Ludwig was raised in Gardiner, Maine and has special interest and training in adolescent medicine and general family medicine.

Neal Norton is a Physician Assistant at Latham Medical Group. He received his Bachelor of Science degree from Rensselaer Polytechnic Institute and his Master of Science in Physician Assistant Studies from the Albany Medical College, Graduate Studies Program. Neal is a long time resident of the Capital Region. He was born in Latham, NY and specializes in primary care. When not practicing medicine, Neal enjoys triathlons (he is an Ironman), is an Eagle Scout, and likes animals, especially dogs.

Shannon O'Meara is a Family Nurse Practitioner. She received her Bachelor of Science degree in Biology from the State University of New York at Albany and her Bachelor of Science degree in Nursing from the University of Miami School of Nursing and Health Studies. Shannon then went on to receive her Master of Science degree in Nursing, majoring in Family Nurse Practitioner, at the State University of New York Institute of Technology. She completed her residency rotation in family medicine, internal medicine, emergency medicine, women’s health and pediatric medicine.

Born and raised in Troy, NY, Shannon has a special interest in family practice and women's health. When she's not caring for patients, Shannon enjoys exercising and spending time with her family.

Cara grew up in Albany, NY and enjoys outdoor activities, like skiing, hiking and camping, when she’s not practicing medicine.
