Our family doctors, nurse practitioner, and staff are dedicated to providing the area's highest quality health care services for you and your entire family. As board certified, licensed family physicians, we are trained to diagnose and treat a wide range of acute and chronic medical conditions for people of all ages. What We Do
Located within the Clifton Park Medical Arts Center, our practice offers numerous advantages over a traditional primary care practice. In addition to our extended hours of operation, we have surrounded ourselves with many subspecialties and services that would otherwise be unavailable at a standalone practice. This results in better care for you and your family in terms of convenience, service, hours, and on-site subspecialty consultation.

NICE Weight Program
Community Care Physicians opened the first weight management program in the Capital Region specifically designed for adolescents ages 11 to 18. The NICE Weight Transitions Program is a weight management and lifestyle modification program that incorporates a health coach, personal trainer, nutritionist and behavioral counselor to help teens and preteens achieve better health.
