After 37 years as a physician, I will be retiring and closing my practice effective August 26, 2011. I have enjoyed the practice of medicine, and I have especially enjoyed the relationships I have established with my patients and their families over the years. Taking care of you and your family has been an honor for our providers and each member of our staff.

I am pleased to inform you that I have made arrangements with Community Care Physicians, P.C. to purchase my practice and to assist me in storing your medical records. Community Care Physicians will help ease your transition to a new physician and will gladly assume your future care.

Community Care Physicians, P.C. (CCP) is the largest multispecialty medical group in the Capital Region. CCP provides primary care and specialty services across Albany, Rensselaer, Saratoga and Schenectady counties, enabling you to stay within one medical group for the majority of your healthcare needs. This multispecialty group also provides access to a free adolescent obesity program for its patients, an on-site fitness center at its Latham location, and utilizes an Electronic Health Record similar to that which you have become accustomed to in our office. Should you ever need hospitalization, CCP providers have hospital privileges at most of the area's hospitals. We are confident that CCP will provide you with a high quality of care, which is demonstrated by their recognition by the National Committee for Quality Assurance, their integration of technology, and their dedication to helping you access your doctor easily both with after hour Urgent Care centers and an online patient portal.

In order to ensure that you continue to receive the appropriate and needed care, I have made arrangements with CCP to help transition your care; however, where you decide to receive care is your choice. Copies of your records will be kept centrally by CCP after my practice closes. These records will be made available to your new physician once chosen.

Community Care Physicians and I have provided a list below of CCP offices with whom you can contact with respect to continuation of your medical care. To establish with a Community Care physician, you can call the office number listed below for the practice you choose. CCP will also contact you to help you select a new physician and answer any questions you may have. The below practices are within close proximity to the location of Bethlehem Family Practice:

This practice offers family medicine services for patients of all ages, as well as acupuncture, osteopathic manipulation, OB/GYN services, and onsite blood draw. Albany Family Practice Group is a teaching facility for Albany Medical Center family medicine residents.

The Office of Drs. Fruiterman, Greenblatt and Maier

This practice provides care for patients over the age of 15. This office offers the convenience of on-site services, including urgent care, lab, audiology, imaging, podiatry and physical therapy. In late winter, this practice, along with the other practices located at 250 Delaware Avenue, will relocate into the newly built Medical Arts Building in Delmar. See enclosed flyer for details.

Dr. Bertam and Dr. Patil provide adult medicine and geriatric medicine services. Located in the same building as Drs. Fruiterman, Greenblatt and Maier, this office also has on-site urgent care, lab, audiology, imaging, podiatry and physical therapy. In late winter, this practice will also relocate into the Delmar Medical Arts building.

Please be advised that should you decide not to receive care from the physicians at CCP, your medical records will be made available to the physician you designate. Since these records are confidential, CCP requires your written authorization to make them available to another physician. If you indeed choose to continue treatment with CCP, there is no need to complete any forms at this time - you will be asked to provide any necessary authorization when you visit the office and your records will be transferred automatically.

If you have your records sent to another provider, CCP will be happy to do this free of charge. Any questions regarding your medical records can be directed to Community Care Physicians, P.C. Customer Service at 518-782-3700.. You can also obtain a copy of your medical record for your own files if you so wish.
